What is the CTGR Program?
CTGR is a chronic disease support program for Aboriginal and Torres Strait Islander people of all ages living with chronic health conditions such as diabetes, asthma or mental health. The team work with clients, their GPs and other health providers to coordinate access to care and improve self-management of chronic disease. CTGR can help with transport, positive lifestyle changes and coordination of appointments.
Who Can Access the Program?
Aboriginal and Torres Strait Islander people of any age who have a diagnosed chronic health condition.
What Clients Can Expect:
Once clients make contact with FocusOne Health, a member of the CTGR team will support them to access a referral through their general practice of choice. Alternatively, a GP may refer clients directly. Clients will work with the CTGR team to develop a care plan that is achievable and meaningful. Clients will receive a collaborative approach to care with general practice and other health professionals involved in their care, such as allied health professionals and medical specialists.

Our CTGR Team
Supports Clients to:
Develop a care plan together that is achievable and meaningful
Attend, support and advocate at health appointments
Access and fund transport to attend health appointment
Fund transport and accommodation costs associated with accessing approved medical specialist services in the Riverland or Adelaide
Fund medical equipment, podiatry recommended footwear or spectacles - for eligible clients
Fill out forms and understand instructions from reception staff
Connect with community-based services such as those that provide support for daily living
Access information, resources and linkages to relevant services
We aim to empower clients to take control of their own health by developing a care plan and linking them with the programs and services to support self-management of their chronic health condition long term.
