About the Role
We are seeking a Cardiac Nurse who will demonstrate a passion and experience in delivering heart disease health services, including assessment and the provision of focussed cardiac strategies to people with a heart disease including Congestive Heart Failure, Ischaemic Heart Disease, Coronary Artery Disease, Heart Attack. The position plays a key role in providing intake assessment, screening, engagement and provision of treatment services to people in FocusOne Health’s catchment in the Murray Mallee, including both individual care coordinate with a Multidisciplinary Team and outreach community education-based approaches.
