We are seeking a Care Coordinator Nurse to join our dynamic and supportive team - someone with a passion for chronic disease prevention and management. The Care Coordinator Nurse may work across a range of general health programs, this may include:
• Closing the Gap Riverland
• Healthy Habits
• Integrated Primary Care Service
• Primary Mental Health Triage
The Care Coordinator Nurse will possess demonstrated knowledge of primary health care services and experience in providing health assessment, screening, monitoring and care coordination support to people with chronic disease and/or at risk of chronic disease. Knowledge of chronic disease management is highly desirable as well as a strong interest and passion for preventative health and promotion approaches.

The successful applicant will be remunerated in accordance with FocusOne Health’s Enterprise Bargaining Agreement (EBA) dependent on skills, experience, and qualifications.
