Enhanced Integrated Primary Care (eIPC)
What is the Integrated Primary Care Service?
The enhanced Integrated Primary Care (eIPC) model delivers coordinated primary care for people with cardiovascular disease living in the Murray Mallee region. A specialist cardio nurse at the Mannum Medical Clinic will work closely and coordinate support for patients through a multidisciplinary team involving general practice, allied health providers and mental health supports.
Who Can Access the Program?
People of any age who live in the Murray Mallee region, and who have cardiovascular disease.
If a client is accessing other services such as My Aged Care, Private Health or NDIS, they may still be eligible to access eIPC services, eligibility will be determined at the eIPC initial assessment.
What Clients Can Expect:
Once connected to the Enhanced Integrated Primary Care service:
The client’s GP and/or Practice Nurse will discuss the Enhanced Integrated Primary Care program with the client
A GP referral will be sent to FocusOne Health
A member of the Enhanced Integrated Primary Care team will contact the client to register for the service
Clients can access care coordination for up to 12 months
Clients will receive a collaborative approach to care with general practice and allied health professionals
Clients will receive Information, resources and linkages to relevant services

This is a free service.
This program has been made possible by funding from Country SA PHN and is therefore unable to cover services under Medicare.
eIPC services are separate from the client’s General Practice Management Plan (GPMP) or Team Care Arrangement (TCA), IPC services are in addition to these services.
