What is the care finder Program?
Care finder provides short-term intensive support to help older people understand their options and access aged care services and other supports in the community. Our care finders can assist with connection to My Aged Care, support at appointments and help with linkages to other relevant services and supports to meet the individual’s needs.
Who Can Access the Program?
Care finder is aimed at people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islanders) who don’t have a support person to help them and are having difficulty accessing My Aged Care and other community supports. It is also available to persons aged over 50 years (45 years for Aboriginal and Torres Strait Islanders) on low income, homeless, or at risk of being homeless.

What Clients Can Expect:
Care finder is available to older people who do not have a support person to help them and are finding it difficult to access My Aged Care and other community supports. This may be because they:
Have trouble reading and writing in English
Find it difficult to understand information and make decisions
Have worries about engaging with Aged Care or Government services
Don’t know what services are available or how to find them
Once connected to the care finder program the care finders will contact the client or a nominated representative and book an initial appointment.
Our care finder team supports clients to:
Connect with My Aged Care and assist through the assessment process
Complete forms and understand aged care service agreements
Connect to other support in the community, such as health and fitness, mental health, housing, and community groups that meet the individual's needs
Our team will also check in with the client every now and then to make sure the services they are connected to are meeting their needs

This service is available in the Riverland and Limestone Coast.
Fees:
This is a free service.
This program has been made possible by funding from Country SA PHN and is delivered in partnership with Country and Outback Health (CoBH).

No formal referral is required, self-referral welcome. Contact the FocusOne Health head office to refer, 08 8582 3823.
